Health Requirements for School entry (Immunizations)

IMPORTANT: TDAP Shot Information for all 6th Grade Entry

  • Virginia law requires 6th and new 7th, 8th, and 9th graders to have a current TDAP shot.
  • 6th grade schedules to be held if no documentation is provided prior to orientation.
  • TDAP shots available for children at age 10 at the Peninsula Health Department (757-594-7410).
  • Your child's physician may wait until age 11 to give this required shot; if so, we must have written documentation from your doctor.

Immunization requirements must be met by all students before starting school. Do not register without proper documentation. All school personnel registering students need to be able to evaluate documentation presented by parents/guardians. Please confer with the school nurse to understand the health requirements.

Whooping Cough for 6th Grade: Virginia state law requires the Tdap booster be given prior to entry into 6th grade. See your private doctor or the Health Department. In 2004, there were 25,827 report cases in the U.S. However, since whooping cough is often mistaken for other illnesses, it is estimated that over 1 million people get it every year. Preteens and teenagers are at risk since the protection from whooping cough shots given to babies begins to wear off after 5 to 10 years.

IMMUNIZATION REQUIREMENTS

Immunization requirements must be met by all students before starting school.   Do not register without proper documentation.

In order for students to attend a school, the following is required by Virginia state law:

  1. A certificate from a licensed physician stating that the student has been successfully immunized against communicable diseases as required by state law at the time of registration.

    DTP, DtaP, DT, TD : OR TDAP:

    MINIMUM 3 DOSES WITH ONE DOSE ON OR AFTER 4TH BIRTHDAY OR
    6 DOSES BEFORE 4TH BIRTHDAY
    1 BOOSTER BEFORE 6TH GRADE
    ALSO REQUIRED IF 7TH, 8TH, AND 9TH GRADER NEW TO NNPS

    POLIO (OPV OR IPV):

    MINIMUM OF 3 DOSES WITH ONE DOSE ON OR AFTER 4TH BIRTHDAY OR
    4 DOSES BEFORE 4TH BIRTHDAY

    MEASLES, MUMPS, RUBELLA (MMR):

    2 DOSES OF MEASLES
    1 DOSE OF MUMPS AND RUBELLA
    ALL AT 12 MONTHS (365 DAYS) OR OLDER

    HEPATITIS B (HEP B):  (Hib is not Hep B)

    MINIMUM OF 3 DOSES

    VARICELLA (CHICKEN POX):

    1 DOSE AT 12 MONTHS (365 DAYS) OR OLDER (IF BORN ON OR AFTER 1/1/97) (School nurse may verify and document parental report of disease)
    GRADES PRE-K, K, 1st, 2nd, 3rd, 4th, 5th, 6th, 7th

  2. Students being admitted for the first time to any NNPS elementary school (Pre-K through grade 5), are required to provide a report of a comprehensive physical examination, signed by a licensed physician or nurse practitioner, performed within twelve months of the initial enrollment date.* Students who have never been enrolled in a school before must have their physical exam completed on the Virginia State Form (MCH 213-F). Attach First Step or kindergarten out-of-state physicals to the MCH 213-F.
     
    • For students transferring into NNPS K-5, a copy of a physical examination in their cumulative record, which meets the above requirements, will be accepted. For Pre-K transfers, only physicals from NNPS PEEP program, First Step, and N.N. Head Start will be accepted. All other Pre-K students must have a physical within 12 months of enrollment date** into NNPS.

    • All students may have a grace period not to exceed 30 school days to obtain the required physical examination. (Note: This does not apply to immunizations.) Students who have not obtained a physical by the end of the grace period will be excluded from school until the required document is provided. While excused absences, this will be referred to Attendance

*The initial enrollment date is the first date of school attendance in any school system (K through 5).

**Enrollment date in NNPS is the first date of attendance, not registration.

By Virginia state law students should not be enrolled without at least one dose of all required shots.
